STATE PROGRAMS FOR SENIORS

The State of Wisconsin has four programs to help seniors to safely stay in their homes. These include Family Care, Family Care Partnership, IRIS, and PACE. Options counseling and enrollment into all of these programs take place through your local Aging and Disability Resource Center.

Family Care

This program helps frail elders and adults with disabilities coordinate their home and community-based long-term care needs.

  • Members of Family Care enroll in a Managed Care Organization (MCO) to receive their services.
  • Members have a care team made up of a Care Manager and a Registered Nurse.
  • Those eligible must:
    •   Be 18 years of age or older with an intellectual, physical or developmental disability OR 65 years of age or older.
    •   Qualify for or receive state Medicaid assistance determined by income and assets.
    •  Meet certain eligibility requirements determined by the long-term care functional screen.

Family Care Partnership

This program helps frail elders and adults with disabilities who have higher care needs to coordinate their medical, health, and long-term care services.

  • Members of Family Care Partnership enroll in a Managed Care Organization (MCO) to receive their services.
  • Members have a care team made up of a Care Manager and a Registered Nurse. A Nurse Practitioner also advises the team.
    • Those eligible must:
    •  Be 18 years of age or older with an intellectual, physical or developmental disability OR 65 years of age or older.
    •  Qualify for or receive state Medicaid assistance determined by income and assets.
    •  Meet certain eligibility requirements determined by the long-term care functional screen.
  • This program is available in the following WI counties: Calumet, Columbia, Dane, Dodge, Jefferson, Kenosha, Milwaukee, Outagamie, Ozaukee, Racine, Sauk, Washington, Waukesha, and Waupaca.

IRIS (Include, Respect, I Self-Direct)

This program guides individuals in self-directing their long-term care services through contracting and invoice services.

  • Members of IRIS enroll through an IRIS Consultant Provider.
  • While members self-direct their own services, IRIS Consultants guide the member and Fiscal Employer Agents also work with the member.
  • Those eligible must:
    •  Be 18 years of age or older with an intellectual, physical or developmental disability OR 65 years of age or older.
    •   Qualify for or receive state Medicaid assistance determined by income and assets.
    •   Meet certain eligibility requirements determined by the long-term care functional screen.

PACE (Program of All-Inclusive Care for the Elderly)

This program combines medical care, long-term care and prescription drugs. It includes the use of community resources.

    • Members of PACE enroll in a Managed Care Organization (MCO) to receive their services. Services are primarily delivered at a PACE center and supplemented with care in the member’s home.
    • Members have a care team made up of their primary care physician, a social worker, and nurses.
    • Those eligible must be:
    • Age 55 or older
    • Eligible for nursing home care
    • Able to live safely in the community with assistance
  • This program is available in the following WI counties: Kenosha, Milwaukee,Racine, and Waukesha.
